Electronic Health Records (EHR) Interoperability
The Federal Government mandate for use of Inter-operable Electronic Health Records (E.H.R.) for Medicare was based on the fact that a fundamental aspect of improving the delivery of patient care is the facilitation of multidisciplinary collaboration and the effective sharing of information between providers. To implement this “Collaborative Care” model, requires management and the provision of smart tools to deliver the right information at the right time and place to the right people. SoftCare’s approach for Interoperable Electronic Health Records provides a comprehensive system that enables the gathering of clinical data from various sources, e.g. existing laboratory, radiology, patient administration, chemotherapy and radiotherapy and follow-up care systems. The system also provides for information on clinical referrals, clinical reviews and courses of treatment to be captured, which results in the delivery of a core patient record that can be shared between all clinicians.
